Foods boosting mental fitness

Stressful life and hectic work schedules have become a part of everyone’s life. These often tend to take a toll on one’s overall health significantly impacting the mental well-being giving rise to depression and anxiety and other similar problems. It is very important to keep these problems in check because if these get aggravated, they negatively affect one’s physical health. What one eats, plays a major role in ensuring their well-being. Also, to feel relaxed and happy one should engage in regular exercises.
Inculcating the following in one’s diet regime can help in boosting the mental health:
1. Foods Rich in Omega 3 Fatty Acids- These are required by the body on a daily basis to reduce the symptoms of fatigue, depression, schizophrenia etc. Omega 3 is an essential fatty acid which is not produced by the body in sufficient amounts. Thus, the intake of external omega 3 sources like walnuts, salmon fish, eggs, canola oil becomes essential.
2. Protein Rich Foods- Proteins act as building blocks for the body and also produce neurotransmitter serotonin which helps in reducing anxiety, depression and other symptoms. Sources of protein that can be included in our daily dietary intake are eggs (white portion of egg), chicken, beans, paneer, nutrella etc.
3. Carbohydrate- Our brain needs glucose which acts as fuel for the mind as well as the body. Inadequate glucose content gives rise to mood swings. Complex carbohydrates are better than simple carbohydrates as they release the glucose slowly and give a feeling of fullness for a longer time. Some sources of complex carbohydrates are whole wheat product, oats, barley, beans etc.
4. Folic Acid and Vitamin B- Symptoms like anxiety, depression, insomnia, and fatigue are related to the deficiency of folic acid and vitamin B. To overcome these symptoms, include green leafy vegetables like spinach, broccoli in the diet. Broccoli is rich in selenium which is a key trace mineral. Deficiency of selenium also effect mental health and it can be overcome by including chicken, whole grain products and walnuts in the daily diet.
5. Fluids- Adequate fluid intake is very necessary for mental fitness. Loss of fluid due to dehydration can result in irrigation. On the other hand, one also needs to be cautious so as to not to over indulge in caffiene rich drinking sources like coffee, tea, soft drinks and the like as these might lead to high blood pressure, depression and many other problems. Thus, drinks like lemonade or coconut water are recommended.
6. Thiamine and Zinc- Deficiency of thiamine and zinc in the body leads to mood swings and mental problems like depression, anxiety, fatigue, aggressive behaviour, high blood pressure. Heavy consumption of alcohol aggravates the deficiency. Some of the rich sources of thiamine and zinc are sea foods, chick peas, eggs, milk, yoghurt etc.

Things You Should Know about Urinary incontinence

Do you lose urine while coughing, sneezing, or lifting something heavy?

Urinary Incontinence or involuntary loss of urine is quite a problematic and an embarrassing health problem. Its severity ranges from an occasional leak of urine while coughing or sneezing to an uncontrollable urge to pee that is sudden and happens just before losing a large amount of urine.

Both women and men can become incontinent due to birth defects, stroke, neurologic injury and physical problems associated with aging. However, women experience it twice as often as men, but WHY? Pregnancy and childbirth, menopause, and the structure of the female urinary tract are the reasons behind the difference. The problem is common, especially in women and can occur at any age but more likely to develop as one gets older.

Normally, when a certain volume of urine is in the bladder, one is aware that the bladder is getting full and it is time to pass urine. The bladder muscle contracts and the urethra and pelvic floor muscles relax to allow the urine to flow out. Incontinence occurs when the bladder muscles suddenly contract or the sphincter muscles are not strong enough to hold back urine. Urine tend to escape with less pressure than usual if the muscles are damaged, causing a change in the position of the bladder. Obesity can worsen incontinence due to its association with increased abdominal pressure.

An unreliable bladder can be disturbing in a number of ways. Here are the main types of incontinence:
Stress incontinence: This happens when one loses urine by exerting pressure to the bladder through coughing, sneezing, or lifting something heavy. Stress incontinence in women occurs with physical changes resulting from pregnancy, childbirth and menopause whereas in men it occurs with the removal of the prostate gland.
Urge incontinence: A sudden and intense urge to urinate, followed by an uncontrolled loss of urine is what it is. Also known as overactive bladder, here the bladder muscle contracts and gives a warning of only a few seconds to a minute to release the urine. It can be caused by urinary tract infections, bladder irritants, bowel problems, Parkinson's disease, Alzheimer's disease, stroke, injury or nervous system damage associated with multiple sclerosis.
Overflow incontinence: When one frequently or constantly keeps on dribbling urine, they are suffering from overflow incontinence, which is an inability to empty your bladder. This type of incontinence may occur in people with a damaged bladder, blocked urethra or nerve damage from diabetes and multiple sclerosis or spinal cord injury.
Functional incontinence: Such incontinence occurs to people with medical problems that interfere with thinking, moving, or communicating may have trouble reaching a toilet.
Total incontinence: When there is continuous leakage of urine, day and night, or the periodic uncontrollable leakage of large volumes of urine, one is suffering from total incontinence.

Urinary incontinence is well reputed of being something associated only with the old ladies but it affects people from all age groups, making it a serious health issue. Fortunately, numerous treatments are in the queue for urinary incontinence. There are certain foods, drinks and medications that tend to cause temporary urinary incontinence. During such situations, a simple change in habits can bring relief. Meanwhile, treating a persistent urinary incontinence depends on the type of incontinence, the severity of the problem and the underlying cause. Behavioral techniques and lifestyle changes are best suited for certain types of urinary incontinence. Under this technique comes the bladder training to control the urge to urinate frequently, scheduled toilet trips for timed urination and fluid & diet management. Often, medications like Anticholinergics, Topical estrogen; Imipramine and Duloxetine are also used in combination with behavioral techniques.

Meanwhile, the physical therapies for urinary incontinence are also considered among the least invasive treatments after the behavioral techniques. Pelvic floor muscle exercises helps in strengthening the urinary sphincter and muscles that help control urination whereas, Electrical stimulation is another procedure in which electrodes are temporarily inserted into the rectum or vagina to stimulate and strengthen pelvic floor muscles. These physical therapies are effective for stress incontinence and urge incontinence.

If all these techniques fail to provide enough relief from urinary incontinence, surgical procedures come to the rescue. A sling procedure uses a strap of synthetic mesh or natural tissue to create a pelvic sling around the bladder neck and urethra that carries urine. On the other hand, a bladder neck suspension procedure helps provide support to the urethra and bladder neck. There has always been a promising viewpoint towards the urinary incontinence treatment as nearly 80% cases tend to improve or get cured, though the best outcome relies on getting the correct diagnosis.

Dengue Fever: You Should Know

What is Dengue?

Dengue or Dengue Haemorrhagic Fever is caused by Dengue virus which is transmitted to man by the bite of the AEDES AEGYPTI. These mosquitoes breed in small collections of fresh water such as masonry tanks, barrel drums, earthenware pots, small or large tins, coconut shells, stored or discarded motor tyres, junk and hardware, flower pots, fire buckets, depressions in free trunks.

What are the symptoms of Dengue?

A patient suffering from Dengue will have two or more of the following:

  • Fever
  • Severe headache
  • Pain in eyes
  • Muscle and joint pains
  • Rash on the body
  • Bleeding under the skin or from internal organs
  • In severe cases, the patient can suffer from Dengue Shock Syndrome which is often fatal

What is the Treatment?
  • Patient should be encouraged to take rest and abundant fluids.
  • Treatment is mainly symptomatic and supportive.
  • Patients with bleeding manifestations and low platelet counts may need transfusion of blood components.
  • Every patient affected by Dengue Fever does not require hospitalization.
  • Patients with high fever, vomiting, rapid pulse rate, low blood pressure, low platelet counts or bleeding require treatment immediately.

What you should do for Dengue control?
  • Dry all deserted coolers & containers in house, offices, factories, schools etc. once a week. Add two table spoons of petrol/kerosene oil in coolers, containers, uncovered/improperly covered tanks, etc. if they can’t be dried weekly.
  • Keep all the water containers/overhead tanks etc. properly covered.
  • Use mosquito repellents on exposed parts during day time to prevent mosquito bites.
  • Use mesh door/windows, mosquito coils, vapour mats etc. to keep mosquitoes away.
  • Use mosquito nets even in the day time especially for infants and small children.
  • Wear full sleeved clothes and long dresses/trousers that cover arms and legs especially during epidemic season to prevent mosquito bites.
  • Use weekly anti-larval methods in and around construction sites where there are water collections.

What you should not do for Dengue control?
  • Don’t let water collect in broken earthenware pots, bottles, flower vase etc. outside or on roof tops.
  • Don’t store tyres in workshops, godowns etc.
  • Don’t give Asprin/Brufen to patient with Dengue/suspected case of Dengue fever.

Breast augmentation

Gone are the days when only women with undersized breasts relied on the breast augmented surgeries for their rescue. With passing time and changing needs, categories of candidates have also taken a noticeable change. Instead of going in for a short term extravagance, women are now ready to throw away their padded bras only for an investment that will benefit them for decades.

Although, a woman may have numerous reasons to opt for breast augmentation surgeries but her motive lies in achieving a beautiful, proportionate and natural appearing figure. Such that, the reasons may vary from cosmetic to medical in nature, and therefore depends on the underlying needs that are being addressed. Notions like - under developed breasts, asymmetrical breasts, pendulous breasts or post pregnancy breasts and Carcinoma or breast cancer reconstruction are some of the many reasons that hamper a woman’s appearance. Thus, a silicone gel implant comes as the best bet to correct the size, form, and feel of a woman’s breasts in a safe and effective manner.

One of the best parts about this surgery is that it is hassle free and requires just an overnight stay in the hospital. Silicone implants are pre-filled with silicone gel, a thick, sticky fluid that closely mimics the feel of human fat. The implants are placed behind the breast tissue in the muscle to avoid any direct touch with the normal breast tissue. The surgery that can last from one to several hours depending on the procedure and personal circumstances is performed under local anesthesia, where the patient remains awake while the surgery is being conducted. It is absolutely pain free without any scars because they are concealed with the breast tissue itself. Therefore, post pregnancy lactating mothers can breast feed their babies without any aggravation.

Besides, the silicone gel filled implants, saline-filled implants are also recommended as they have a strong, silicone shell and are filled with a sterile salt water solution. They are either pre-filled or can be filled through a valve once they have been inserted into a breast. Saline implants can be considered, as they have a long history of safe use and because the saline solution is similar to the body’s fluids, it can be safely absorbed or gotten rid off by the body if the implant ruptures. A woman needs to be at least 18 years of age to get her breast augmentation surgery done with saline-filled implants and at least 22 years of age to receive silicone implants. The cost of a surgery will also vary according to the type of implant ranging from anywhere between Rs 80,000 to Rs 1.25 lakhs.

I remember a young girl visiting me who was going through depression because of her small breasts. She was beautiful, smart and elegant but this problem kept nagging her making her feel insecure and unhappy in life. She was of a marriageable age and was very skeptical of getting a suitable partner for herself. Moreover, the topic was too embarrassing for her to discuss it with anyone. She started reading about it and came across certain products that claimed to be helpful in breast augmentation. But she was deeply de-motivated when she realized that these products were not working. She then finally decided to go under the knife but was very hesitant to do so. She was doubtful about the efficacy of breast augmentation surgery. However, our team of doctors counseled her about the advantages of this procedure and assured her of the safety of this surgery. Now, when she has been successfully operated upon, she is a more confident and happier person.

So, I would suggest that before going forward for any surgery, consulting a specialist surgeon should be on the priority list. Whereas, the surgeon should be able to discuss whether the candidate is suitable for breast implants, the different type of implants, options for size, shape and placement, as well as the risks and benefits of implant surgery. The surgeon should also be able to provide them with ‘the before and after pictures’, for a better understanding on the potential outcomes from the surgery.

Following a breast augmentation surgery, the patient can be discharged a day after with no requirement of safety measures, but one needs ample amount of rest for a speedy recovery. For the first few days, it’s normal to experience a feeling of tightness in the breast area as the skin adjusts to the new breast size and silicone implants. A patient may also notice a diminished feeling in their breasts and nipples during immediate recovery but a regular breast massage along with some exercises will help alleviate any discomfort. In fact, most patients are able to return to work within a few days after their surgery. However, it is best to avoid any exhausting activities for the next few days that could raise the pulse and blood pressure or that require strenuous use of arms and chest.

With an advanced technology, surgeons are capable of performing the procedure so well, that the results are natural looking, much to the delight of every women looking forward to give it a try. So, getting that perfect breast shape will no more sound complicated and risky!


Gynecomastia at a glance

The term comes from the Greek words Gyne meaning woman and mastos meaning breast. In practical terms, this means abnormally large breasts in men.

The condition is relatively common in adolescent boys, and 90% of the time symptoms disappear in a matter of months, or as adolescent wanes, a few years later. But the remaining 10% are burdened with a social handicap that causes a deep and complex shame, and puts one’s relationship with one’s body at risk.

Gynecomastia affects aprrox. 40-60% of the male population. This is the most common cosmetic procedure done on men worldwide. Public embarrassment while wearing tight fitting clothes, going to the gym, for swimming, peer pressures are the reasons why patients consult a plastic surgeon. Gynecomastia when treated surgically brings back patient’s confidence.

Surgical intervention usually involves liposuction for the fatty tissue as well as excision of glandular tissue. Rarely does it happen that Gynecomastia is purely excessive fatty tissue and all that is required is liposuction.

The surgical procedure can be done under local anesthesia wild mild sedation or general anesthesia.

Infiltration of the breast is done with xylocaine and ringer lactate using tumescent technique and two small ports are made on either side for liposuction. Then the gland is excised through an intra alveolar incision. Finally, liposuction is done again to smoothen the periphery. Fine sutures are placed for skin closure and bulky dressing is done. Compression garment is used for four week to keep the cavity collapsed and early recovery of skin texture. Patient remains fairly alert and can go home the same day.

Most patients are back to work within 2-3 days and can drive their cars after a week. Heavy exercises and weight lifting should be avoided for 3-4 weeks. Patient can start taking shower after 3-4 days. Stitches are removed after a week. The pressure garment is easily concealed under normal clothes without getting noticed.

Complications are rare in such surgeries. Scars are almost negligible as the incisions are small. Sometime there are minor irregularities like some fluid collection which may need to be drained , but this happens in rarest of rare cases.

Gynecomastia surgery’s results are permanent; however, weight gain or use of certain drugs may cause the condition to reoccur. Men do find it very difficult talking about their breasts to anyone, but it is the first step towards relief. Realizing that they are not alone is a powerful antidote for the shame and beginning towards healing.


Listen to your heart before it is too late!

As 30 year old Barkha Duggal suddenly started gaining weight, her husband advised her to join a gym. While exercising, she used to feel a strange uneasiness. However, she was told by her friends and relatives that her uneasiness would subside as she gets used to exercising. But with time, her problems kept increasing. On examination, she was found to suffer from high B.P. and cholesterol. Since there were no cases of cholesterol, high B.P., diabetes or obesity in Barkha’s family history, she was very sure that she would never suffer from any heart ailment. Only after the doctor insisted did she go for an angiography wherein it was detected that she had 90% blockage in the artery of her heart.

There are many urban youths like Barkha who are unaware of the fact that they too are vulnerable to various heart problems. It has been observed that the haphazard lifestyle led by today’s youth is the root cause of all problems. Stress at the workplace, long working hours, eating out, having meals at odd hours, lack of any physical activity and a callous attitude towards their body is taking a toll on the heart. At times, it can also lead to fatal consequences.

Its time to take an action if one notices the following symptoms:

  • Shortness of breath
  • Palpitations (irregular heart beats or skipped beats)
  • Discomfort or pain in chest , arm and wrist in times of stress
  • An uneasy feeling in the throat (as if something is stuck in the throat)

In the light of all this, it is very essential to bring some changes in one’s lifestyle so that the risk of heart diseases can be reduced to a minimum. Following are certain tips that can be put to practice:

  • Engage in any sort of physical activity on most days of the week.
  • Eat a healthy and a balanced diet (reducing the use of high fat foods).
  • Avoid smoking and the intake of tobacco.
  • Get regular examination done of both high and low blood pressure.

However, not all heart ailments are as serious as Heart Attack. Broken Heart Syndrome (BHS) or stress cardiomyopathy is a temporary heart ailment that is often misunderstood as massive heart attack. It is generally triggered by extreme and sudden emotional trauma such as death of loved one, divorce/break-ups, losing one’s job and even the shock of a surprise party. In BHS, a part of one’s heart temporarily enlarges and doesn’t pump well, while the rest of the heart functions normally.

The symptoms of BHS are:

  • Tightness in the chest
  • Loss of appetite and sleep
  • Feelings of loneliness, hopelessness and complete emptiness
  • Constant or frequent crying
  • Feeling of hatred

A ray of hope lies in the fact that this condition is just a temporary condition and is usually reversible in about a week. It mostly requires the effort of the individual suffering from BHS to take charge and get over the condition and resume leading a normal life.

Our heart is one of the five vital organs and it needs to be taken special care of. Even a subtle indication towards its malfunctioning should set alarm bells ringing and before it is too late, one should take appropriate measures. Regular check ups and visits to the doctor should be made a part of the health regime.


Gestational Diabetes

Pregnancy is an exciting time in a woman’s life- a time of expectations and preparations. Taking care of the mother’s health during the gestation period is of utmost importance for the baby to grow normally and enter this world hale and hearty.

Sometimes, especially around the 24th week of pregnancy, many women are diagnosed with high blood sugar- a term called Gestational Diabetes, even when they have had no history of diabetes. The diagnosis of gestational diabetes does not mean that the mother will remain a diabetic after delivery, although a susceptibility to develop diabetes in the future is certainly present.

We are all aware that during pregnancy, placenta supports the baby as it grows. Placental hormones are necessary for the baby to grow to and sustain the pregnancy, but these hormones also block the action of insulin in the mother’s body- a term called insulin resistance. So, although the mother’s body continues to produce insulin, it is not allowed to work properly and as a result her blood glucose levels start rising. Gestational diabetes develops when the mother’s blood glucose levels rise above normal, thus, causing hyperglycaemia.

Gestational diabetes occurs at a later stage in pregnancy- when the baby has already formed but is still growing. As a result, though the baby may not have developmental birth defects but if it is left untreated, the baby might be harmed. Rising blood sugar levels in the mother’s body send the pancreas into overdrive to produce more insulin, causing hyperinsulinemia, yet this is not able to reduce the blood glucose due to increasing insulin resistance caused by placental hormones. The insulin cannot cross the placenta but the extra glucose can easily enter the baby’s circulation through placenta along with other nutrients. As a result, the blood glucose levels of the baby also start rising, sending the developing pancreas in the baby’s body into an overdrive to produce extra insulin to control them. As the baby receives more glucose, i.e., more energy than it normally needs for growth, this excess energy gets stored as fat in the baby’s body, thus, leading to ‘macrosomia’ or a ‘fat baby’.

Treatment for gestational diabetes should start immediately when the mother is diagnosed with high blood sugar levels after undergoing an Oral Glucose Tolerance Test. A good treatment regime aims at keeping the blood glucose levels at normal throughout one’s pregnancy. It starts with special customized meal plans for the expecting mother as well as scheduled physical activity. The blood glucose level needs to be monitored on a daily basis as well as insulin injections need to be taken if dietary and lifestyle measures alone are not able to normalize the blood sugar level. During pregnancy, doctors prescribe insulin injections and not oral tablets because in pregnancy all oral tablets cross the placenta and can harm the baby but insulin does not cross the placental barrier and, therefore, is very safe.

Gestational diabetes usually goes away after pregnancy, but there are chances that in 2 out of 3 cases, it might return in future pregnancies too. In fact, in a small section of women it may unmask type 1 or type 2 diabetes! Such women may need to continue their diabetes treatment even after pregnancy. Losing a few kilos of weight, adopting healthy eating habits, exercising regularly and staying fit; go a long way in preventing future diabetes.
